Cervicogenic Headache Treatment in Boise: Is It Your Neck or a Migraine?

What Is a Cervicogenic Headache?

The word cervicogenic simply means “coming from the neck.” A cervicogenic headache is pain that is referred up into the head from structures in the upper neck — the joints, muscles, and nerves near the top of your spine. The brain feels the pain in the head, but the source is lower down.

This is why the top two bones of your spine, the atlas and axis, are so important. They sit right below the skull, and when they are out of balance they can irritate the nerves that feed into the head. We explain why these two bones matter so much in more detail.

Cervicogenic Headache vs. Migraine: How to Tell the Difference

There is some overlap, and the two can even occur together, so this is a guide rather than a diagnosis. Still, a few patterns tend to set them apart:

Cervicogenic headacheMigraineOften starts at the base of the skull or in the neckOften felt deep in or around the head, sometimes one-sidedSteady, aching painThrobbing or pulsing painWorse with neck movement or sustained postureWorse with activity, regardless of neck positionComes with neck stiffness and reduced range of motionComes with nausea and sensitivity to light or soundRarely involves auraMay involve aura or visual changes

If your headache eases with rest or gentle neck movement, flares after a long day at a desk, or travels alongside neck pain and a stiff neck, there is a good chance your neck is involved. A history of whiplash or other head and neck trauma makes that even more likely.

What the Latest Guidelines Say

The idea that the neck can cause headaches is gaining real recognition. In February 2026, a clinical practice guideline developed by a 57-member expert panel and published in the Journal of Integrative and Complementary Medicine recommended spinal manipulation as a primary, nonpharmacological option for cervicogenic headache. That reflects a broader shift in the profession toward taking neck-related headaches seriously.

We want to be precise about what that means. The guideline supports hands-on care for cervicogenic headache in general; it is not a statement about any one clinic or technique. Upper cervical care is its own gentle, specific approach, and the research on it is more limited and largely case-based. We share these guidelines as a sign of growing professional recognition — not as a promise of any particular result.

The Upper Cervical Approach at Peak

Upper cervical care is a focused branch of chiropractic that works only with the top of the neck — with no twisting, cracking, or popping. When headaches are tied to an irritated upper neck, calming that area gently is the whole idea.

Your visit begins with precise measurement, not guesswork. We use 3D CBCT imaging and functional nervous system testing to see exactly where and how your atlas has shifted, then use low-force, specific methods such as the Blair upper cervical technique or the knee-chest technique to guide the bone toward its natural position. For more, see our companion guides on cervicogenic headache relief and how upper cervical care relates to migraines.

Everyday Habits That Help Headaches

So much of headache relief lives in daily habits, especially for neck-related pain. A few things Dr. Kyara often talks through with patients:

  • Fix your screen posture. Hours of forward head posture are a classic trigger for neck-related headaches. Our article on posture and tech neck shows simple ways to set up your day.
  • Support your neck at night. The wrong pillow keeps the upper neck strained for eight hours straight. A properly fitted pillow can make a real difference — see our post on ergonomic pillow benefits, plus supportive options on our Amazon storefront and a quality mattress from Saatva or Avocado.
  • Cover the simple triggers. Dehydration and skipped meals are common, fixable culprits. Our guides to hydration and anti-inflammatory foods are good starting points, and for clean staples we like Just Ingredients.
  • Calm tension at the source. Magnesium supports healthy muscle and nerve function and is one of the first supplements we reach for — find practitioner-grade options through our Fullscript dispensary and read more on magnesium and nervous system health. Gentle recovery tools like Hooga Health red light or a Nurecover device (use code PEAKCHIRO) can help you unwind, and targeted massage from a practice like Cohen Injury Massage can release tight neck muscles.

None of these replace care, and none are a cure. They simply remove some of the daily load that keeps headaches coming back.

Most people think of water as something to drink when they're thirsty. But for your nervous system, your brain, and your spinal cord, water is a constant necessity. Dehydration — even mild — can reduce cerebrospinal fluid flow, slow nerve signaling, and trigger headaches and brain fog that look and feel like neurological problems. Here's why hydration belongs at the center of your wellness routine.
